Cheeks and Tear Troughs
When we are young soft tissue fullness creates a smooth, almost imperceptible transition from the cheek to the lower lid. As we age this fullness, which is mainly fatty tissue, shrinks and drops, thereby showing the muscles (causing darkness) and ligaments underneath. These ligaments become more prominent and result in grooves under our eyes known as the tear troughs.
The tear troughs and the loss of fat in the upper part of the cheek below the lower eyelid creates an ageing tired looking appearance. On treating both the tear trough and the fat loss in the upper cheek with the correct and very accurate placement of filler, a natural youthful appearance can be restored.
By filling the cheek area just below the lower limit of the tear trough not only does this refill the cheek in this area but also raises the lower limit of the tear trough thereby giving a younger less tired look. Once this has been done the tear trough can be filled directly. The combination of both treatments, which can be done at the same time, restores a natural youthful look and removes the tired eye look.

On the picture below I have marked lower and upper end of tear trough and the red outline is where the fat has been lost and the cheek now has a flat, ageing appearance, unfortunately, this happens to us all as we age. Fillers can dramatically improve this area.
Unfortunately, some inexperienced operators overdo this area and patients ends up looking unnatural. Good example is show in the picture of Nicole Kidman below. I do not do this as I am always after a natural look for all my patients.
